Cleanup presentation of theme_file_upload_help()

* file helper tips are formatted by <br> leaving little room to style, and make readability difficult
* "allow file format" is often confusing to editors, especially when using an image.
* add collapsible fieldset as wrapping to reduce the visual space of items within.
