@suite117, thank you for this report.

I tested with simplytestme and Drupal 8.9.11 and a standard profile. Install content_translation and added Italian, enabled translation for the article content type. Then added an article with an image. I translated the node and was was able to translate the node and the alt text for the image.

Therefore, closing as cannot reproduce. If you are experiencing this problem reopen the issue, by setting the status to 'Active', and provide complete steps to reproduce the issue (starting from "Install Drupal core").
