It would be useful to provide an option to use the current path for "form action path". This would allow the block to be used across multiple pages and would also allow it to be used on views with contextual filters.

Worth mentioning - I found the multiblock module to be useful for duplicating the date picker block, in case anyone else is looking for this functionality.
