I had performance issues with large node grants queries (> 150.000 chars each) on larger websites in combination with Node access node reference and developed https://www.drupal.org/project/content_access_booster. You can find details about the WHY and HOW on the module page.

If the maintainer is interested we could merge this into Content Access.
