Problem/Motivation

The form elements do not render form prefixes.

Steps to reproduce

1. Using the form API, add html or text '#prefix' or '#suffix'.

Expected outcome prefix and suffix being rendered.

Proposed resolution

1. Add variables and place to output prefix and suffix.
