What does "Let the site determine which avatar to use" actually mean?

What avatar should the site be using if the user has "Let the site determine which avatar to use." installed? I assumed that it would fail over to the user selected option which I have a default set up but it is not showing. How do we go about making sure a user always has a avatar so that we can make sure formatting is correct in our HTML?
