My Default Dupral 7 Toolbar looks as such:

But once I disable the Toolbar module & enable the Administration Menu Toolbar this is what I get:
Drupal 7 Admin Menu Toolbar Fail

I've tried to disable/uninstall/delete the module & then re-downloaded it, installed it. Unfortunately this doesn't resolve the issue. For now I've disabled the module until the issue can be resolved. Please let me know if there is any other information that would be beneficial to help me reach a resolution.

Oh and also to note: I intentionally blurred out the username in the second screenshot, hence why it appears solid red. Not sure why I blurred it out, but anyways just wanted to clear that up in cause it causes confusion :)

I've recently noticed that there are duplicate menu items in the admin/structure/menu. I've been able to disable some of these menu items manually, but some menu items that are in the admin menu are not listed at all to be disabled.

So, it looks as if something else is triggering duplicate menu items, and admin menu is displaying these items outside their appropriate categories.

I noticed these changes after upgrading to 7.18.

I wish I would be able to get some time to figure out how to disable these manually myself :( I've since disabled this module, but boy would it be nice if this module was actually supported...

11 weeks without any response....

Fortunately I found that by uninstalling and removing the remnants from sites/all/modules for 2 months, paired with minor core update to 7.19 I was able to finally have a functional version of this module with version 7.x-3.0-rc4.

My guess is that someone discovered that the module wasn't release ready and was trying to sweep things under the carpet rather than admit their failures...

This worked for me:

DELETE FROM menu_links WHERE menu_name='management'

Followed by a cache clear.

Note that #13 works but will obviously remove any custom changes you have made to the management menu. It basically resets it and works with any menus where you have problems.

I am sorry for no reply until now.

There are many issues regarding this module admin_menu which is a bit difficult for us to follow up since some of the issues might be already outdated, or is already fixed by the module or any other modules or itself core which means that the problem might no longer need to be fixed.

We can see that the issue has been created for a few years ago, I hope it is okay for you that I am postponing the issue, and give you around two weeks. If you still face the problem, could you tell us the step by step when until you get the error message or what is frustrated you, and a list of modules you are using related to admin_menu and a screenshot that might help us? So it makes us easier to reproduce your issue.

However, after two weeks with no feedback - we will close this issue. So in case, you noticed it after the issue is closed, do not hesitate to reopen it like and fill information which is mentioned above.

So before giving us a feedback, do you mind to test it again with our latest 7.x-3.x-dev?
