When changing the Country the input of checkboxes can return unwanted values.

Steps to reproduce:

  1. Have a checkbox form altered into the form
  2. Leave the checkbox off (default value 0)
  3. Change the country
  4. Verify the checkbox is checked

Desired outcome:

  1. Have a checkbox form altered into the form
  2. Leave the checkbox off (default value 0)
  3. Change the country
  4. The checkbox should stay unchecked
