is it possible (and if so how would i go about) to put a title above several checkboxes with the profile module? as an example...

Favorite Foods (select all that apply)
[ ] pizza
[ ] pasta
[ ] cheese
[ ] steak
[ ] other

a user would be able to select all that apply and 'Favorite Foods' would be the title for the check box list.

i'd be happy to use another module to gather profile info but i need to be able to export and email a csv file of the profile.

any thoughts? thanks for your help!
