I have tried several key words but find no related information. Please point me to the thread if it is a past issue. What I want to do is a form with 5 sets of fields containing details for members of a family. But I would like to have a button for user to add more if he has more than 5 family members.

Also, I want to know if webform can do this:
For a multiple choice question, if user click on an option, a textfield appear for user to fill in details.
