$parents is not used further, so simplify code by assigning array() directly to #parents rather than using an intermediate variable.

Reverse order of test for rebuilding $cache. Looking through the code in form.inc suggests that this function will be called more frequently without the refresh parameter set. Therefore we put the most common test (that is, whether $cache is set) first.
