Where does it come from? What is this type of icon (beside the url) called?

in the base directory u can find favicon file. u can change whatever u like...

its a windows standard, however, not all IE browsers represent the icon in all situations (some do in the bookmarks, some in the url...) note that most non-ie browsers will render it nicely! :-)
