web form not producing emails correctly

I have a number of webforms in place on a variety of sites. They all generate the corresponding email with all fields correctly- except this one form that I have repeated recreated and have the same results- the email generated does not have the form elements included for viewing in the email, but they are recorded correctly under the results tab.
The settings and process I have used to set up the webform in question is exactly the same as the others I have set up that work correctly. Please advise, and look at the form if you need to (www.mesarvresort.us/reservations)
