Problem/Motivation

We are currently supporting custom file browser but we should also support custom checkboxes/radios and custom select menu. See https://v4-alpha.getbootstrap.com/components/forms/#custom-forms.

Proposed resolution

Add support for checkboxes/radios and select menu. Also, introduce custom_forms theme settings boolean so custom forms can be enabled or disabled. By default enable it.
