I have Workbench set up with about 6 unique States and all the Transitions in between the various states set up.
When any node is progressed from Draft to any other transition, that revision is published. Even if there is already a published revision.
This seems like it's a huge mistake somewhere. Am I setting up something wrong? Could there be a module interfering?
I realize I probably need to provide more information but I really have absolutely no idea what could be causing this. I don't even know where to start.
Has anyone ever seen anything like this before?

nesstheheroIn addition, "Rejecting" the node back to Draft completely unpublishes any revision that was published and now the page is completely removed from the site. Anyone accessing it gets an "Access denied" message.
