In the WYSIWYG, if you insert an image and set an alt or title, it's impossible to clear the alt or title on subsequent editing - it just reverts to the last value.

The referenced Media patch has been included in its new stable 2.0 release, which Panopoly updated to in #2869079: Update to Media 2.0. This patch is no longer needed.
