Error message - Notice: Undefined index: input_type

I had some slideshows working on various pages, then when I added another page to display the slideshow block on (under "configure block" "show block on specific pages"), I received the following message:

Notice: Undefined index: input_type in ddblock_block_save() (line 201 of C:\xampp\apps\drupal\htdocs\sites\all\modules\contrib\ddblock\ddblock.module).

Now the slideshow doesn't show up on the pages I specified.

What does this mean and how do I fix it?
