After update to beta3 (and beta4 later) ability to override title/alt values for images inserted inside TinyMCE textarea was lost. Looking at media_wysiwyg source found that create_element method doesn't use 'fields' property of inserted media object. The problem has to be investigated through, simple patch for quick fix will be attached in first comment.

did some digging to get to the bottom of this.
this was caused by a regression between patch 71 and 66 in the related issue

Taking your fix, it basically takes us back to patch #66 which had positive feedback for last few months.
