list members of organic group with text box in front of them

Hi.I have a organic group.I want group members in a form with a text box is displayed. So that a score is given to each group member.

After you have created the group ,automatically, In a list with a text box in front of each member of the group to be displayed And to be in the text box, enter the member's score.
How can I do this?
