I installed and enabled this module. Even after clearing cache, if I go to edit a content type, the theme is disabled on that page and I don't get any form settings. See attached screen shots.

I did exactly what you said but it's working fine at my side. I am using drupal 7.50 and bartik theme. Can you please let me know which theme you are using?
