Pathologic works fine when adding normal content. In custom blocks, however, the paths remain relative.

Can't replicate - the links in my test custom block are being processed by Pathologic.

Are you sure your block is using a format which has Pathologic enabled?

I'm willing to bet the custom block is generating markup directly, not invoking a filter, and therefore pathologic has no chance to do anything.
