There will not be any security problems, and if your sites do not use 4byte characters, and never will, you will not have any troubles with them either.
It sneaks up on you
I want to enable this now that I know about it because I have problems caused by four-byte UTF8 characters, which you cannot see in the input AT ALL, causing webforms to throw errors and preventing users from submitting. The user has always been Japanese, using a Japanese OS and pasting from something else, maybe PDF or Word. The error messages I was getting led to Japanese MySQL forums where the users were talking about incompatible four-byte UTF8 characters.
