Adding the action 'Create a new entity' shows a form with a duplicate form field, and the action does not fire.

However, still strage that the create action form shows a duplicate field for 'bundle'.

Anyway, I tried the "Create a new entity" action on all 14 of the entity types defined on my system, and did not see the duplicate field that you complain about. I suspect this problem is specific to the module/entity that you're using - perhaps the entity definition is wrong somehow.
In order to do something about this, I need to be able to reproduce the error. So if you can reproduce the error using one of the core entities, or tell us what module you are using which defines a "Domain" entity, then I can look into this further. Otherwise I will have to close this as "Cannot reproduce".
