Problem/Motivation

When the entity create form is being presented in a ctools modal, then the visual_select_file's jquery ui modal gets hidden behind the ctools modal, and hence making it unusable.

Proposed resolution

Increase the z-index of visual_select_file modal to be greater than ctools modal.

Provide patch to fix the issue.
