Layout of Telephone (grouping Place & Tel No side by side) with Field Group Multiple

Hi,

I want to have a Layout of Telephone with 2 fields (Place & Tel No) as follows

I tried Field Group Multiple but causing troubles with Field Collection in data saving.

Any idea ? thanks
