HOWTO: Assign meta tags to specific (non-entity) pages (D7)

There are several ways of defining meta tags for individual pages, depending upon how it's built. Pages that are controlled by an entity, i.e. nodes, taxonomy terms, users, etc, should be handled by the meta tags for that entity. Other pages can be handled a few different ways, depending upon the situation.
