I think the issue is fixed but not for multi language.
I have created a patch but I must admit I don't know if it is a good route to go.
I don't have the overview on the codebase of pathauto.

I still get /title-0 and /title-0 for a node in different languages where the title translated is the same.

If someone could review the patch I would happily improve it.
