I have a function that have this

I have a .tpl file where there are this
<form action="<?php print $base_path.'add-user-to-branchOffice' ?>" method="POST" >
<label>BranchOffices</label>
<?php print $select_header; ?>
<input type="submit" class="form-submit" value="Save">
</form>
but the button submit don't work.
and if I remove the variable form works correctly.
I only wish I could have on file .tpl the label form and send items created by the Hook_form to the .tpl file
