It looks like php-fpm was not acting correctly on an www7. This is resolved.

Can extra monitoring be added to catch these faster?
This has been going on for at least 7 hours without @drupal_infra being aware.

To be clear, this wasn't anything to do with Fastly, other than it making a nicer error page. Fastly support actually proactively reached out to us to ask if they could help.
