Same issue here.. I applied the security patch and changed commerce_hdfc.install found in the issue queue, but no avail.
I am not getting an option for entering Card number and CVV number when I select "Commerce HDFC Payment Gateway" in review order section.
I continued to next step and got error.

EntityMetadataWrapperException: Unknown data property commerce_firstdata_billing_phone. in EntityStructureWrapper->getPropertyInfo().

Please note:
Commerce HDFC Payment Gateway is based on the previous Dual Verification method and not on the latest Hashing Integration (i.e, with Hashing Flag Disable).
