A few notes:
a) The comment at the top should be removed that says what the topic covers.
b) I think you're right about the index entries under Entity. Maybe also some that start with "Field"? But I'm not sure we need both "adding" and "creating" entries, since they'll be one right after the other in the index, maybe just things like:
Reference field, creating and adding
"Node reference field" might also be a useful entry for those coming from previous versions of Drupal, and "User reference field" too?
c) Start of Steps, first intro sentence, maybe start it with "Here are the steps to..." and end with : so it is not confused with being a "step" per se?
d) I can't think of any related or d.o references, so we can probably take out those @todo notes.
e) Needs screenshots
Anyway the text looks pretty good to me, just those minor comments... so probably ready for screenshots now! Thanks!

batigolixI fixed the points mentioned in #4
Regarding point b): I also added Taxo term reference as index entry. And maybe adding&creating is a little bit too much of the same so I used "adding" according to the topic title.
Regarding d) do you know of any related concepts in the guide? If not you can remove the @todo
