However, this bug doesn't happen on a fresh install of Panopoly 1.26, at least not in my testing. You can do a quick install using simplytest.me at this link:

https://simplytest.me/project/panopoly/1.26

It was something that happened in an old -dev version of Panopoly (see #2245133: Media Browser appears underneath the CTools dialog) when we attempted to update jquery_update, but it was fixed when ultimately merged #2235081: Update jquery_update to version 2.5

So, I think something on your site (possibly related to the Media or jQuery Update version) might be interfering with the fixes we did there. Can you check that you don't have a different version of those modules than what's included in Panopoly 1.26?

However, if you are able to get this bug to happen with a fresh install of Panopoly 1.26, please give steps to reproduce!

Not at all same module versions. This happened after upgrading to 1.26, so I just assumed...
