"Remove User Role" Action resubscribing User to OG group after "User has been removed from Group"

The Action "Remove User Role" resubscribes a user to their group once they have been removed from the group. I have attached pictures here to show as examples.

Picture #1 is the rule that I have.

After I click the "Remove User" button it gives me picture number #2 as a result. Having "Test Add" resubscribed twice.

I reset everything manually and repeated again executing a batch command to remove users, picture 3 is the result. Having "Test Add" resubscribed only once.

I can't figure out exactly what is causing the issue or how to solve it. Any immediate help debugging this would greatly be appreciated.
