how to add a custom external link to the image field

I like to know how to add a custom url link to the image field in drupal's core when i create a custom content type, a module called "simplead" does this for its module, i provided the screen shot below

Don't think there is a "native" way to do it. The closest I could think of is probably installing the field collection module.

It also rather sounds like you should address this in the display than in the data structure, as it looks like you would only have 1 image + 1 link. So no reason to combine them anyway.
