In grid components, questions should be in th elements. The questions are really the headers for those rows of options. The questions should have @scope="row" and the options @scope="col".

While semantically true, many themes are designed with th elements assumed to be column headers. For exampe, the them I used defines the style as 'th' rather than 'thead th'. This may be necessary for tables that aren't themed with thead elements.

I don't see how we can do this in 7.x-4.x, so let's move this to D8.
