For reasons I've not been able to fathom, $view->get_items_per_page() sometimes returns 0, even though the pager is set to a positive number.

At any rate, views_calc should treat a 0 value here as meaning 'the pager is showing unlimited items', and therefore there is no paging, and no page subtotal should be shown.
