As a test, I've installed BASIC theme on test server with no changes.

I cannot get any block to display in left or right hand column.

Surely this is something I'm doing wrong, rather than any bug.

Is there something extra that I need to do - other than assign the block to the appropriate region?

As I thought, it was me. :-D

It's either something I've done wrong with themekey or misconfigured in the block. It's working on my homepage when I switch themes so that's good enough for now.
