Extensible form wizard won't go past first page [#2304787]

I've got a multi-step form that i created using the extensible form wizard. The form works normally on my acquuia dev desktop, but on the live site when i click next it just reloads the same page. As a test i copied the entire code of the example without any changes and the same thing happened. On my live site the code does work, but ONLY when i run in the examples module, not in mine. I've checked everything that I know, and my code looks identical.

Any advice would be much appreciated, I've been trying to solve this for over a week now.

I'm a little unclear on this... Does the form_example module work on both your local and live site?

Do the form_example tests pass on your live site?

Yeah the example module works fine on both sites. It is when i run that same code from my module on the live site that the page won't advance. The same code DOES work when i run my module locally. I've uploaded my module file and the .inc for this form. This is the code that works locally but not on the live site.
