Open Entity Reference Link to New Tab or Window

My site uses Entity Reference heavily. When user clicks one reference item, I need it to open in a new tab or new window. I understand it is not recommended, but it is really frustrating for users to go back and check another reference item. I have searched web but didn't find clear solution. I understand that there is one issue for the same request reported How to change the link target in a entity reference display? over two years ago, but I have trouble to understand the solution in reply. Could anyone kindly tell me how to modify the code in this module or in the theme?

I have been searching for solution for months. I'd greatly appreciate for your help...
