Duplicate url records are created for each subscriber

Only new url records that do not have a matching 'nid' and 'url' should be stored in the database. Currently duplicate records are being created for each subscriber.

E.g. A newsletter with four links and a thousand subscribers should have four url records. Currently that newsletter would have four thousand records created.
