Lacking (most) configuration options and one permission

I installed RealName and it appears to be working (names displaying as I would expect them to), but:

1) at /admin/config/people/realname there is only one configuration option (Realname pattern), and none of the others mentioned in the documentation are present,

2) There is only one permission - Administer Real Name, whereas the documentation says there should also be one for 'use realname'.

I feel like I must be missing something obvious, but can't see what it is. Thanks!
