I've enabled flag, but in display suite / manage display I cannot see any field for the flag module.
Am I missing something?

You need to enable the "flag integration" inside the "extra" tab of the ds module.

1) Enable ds_extras
2) Go to admin/structure/ds/list/extras and find the flag option
3) Enable it
