The attached patch:
- cleans up some very minor typos found in the code comments
- standardizes on some common documentation conventions: CCK, HTML, etc.
- modifies some formatting in UPGRADE.txt to enhance readability

Very minor stuff.

I thought I might help out with some hook_help documentation as soon as I have the opportunity, but thought I would start with this.
