I have a strange behavior on a site in which I use fast404, leaving the default configuration should exclude the .asp pages, but don't works on all paths.

For example it works with:

http://www.ecodelchisone.it/articoli.asp

but not with:

http://www.ecodelchisone.it/articolo.asp

Why some paths are not excluded from standard 404?

I can't replicate this. A guess is that you have 'physical files' in your docroot - ie articolo.asp. These will not hit Drupal as instructed by the standard .htaccess.
