I am under the impression that custom formatters should show in the Manage Display tab for content types, but they don't. They appear for views though.

Am I mistaken, or are there some settings I've overlooked?

They absolutely should, the only reason I can think that they wouldn't would be that you don't have any Custom Formatters for the fields that are being displayed on the Manage Display tab, but given that you say they show up on Views then it's unlikely to be the cause.

It's possible that another module is interfering, or it could be any number of things.

Can you provide some screenshots to show the Custom Formatters overview page, the Formatters being shown in a View and the Manage Display tab so I can see if there's anything obvious?

I am getting this problem as well. I created a custom formatter but I cannot find it under MANAGE FIELDS or MANAGE DISPLAY.
Using Custom Formatters version 7.x-2.2 and Drupal version 7.20 (Acquia Drupal (acquia-7.x-1.19))

I just tested with the latest Acquia Drupal and latest Custom Formatters dev and there is no issue.

Possibly just a misunderstanding on how the module works, the created formatters are only available on fields of the type assigned in the Custom formatter, so you would only see a formatter made for an Image field on an Image field.
