It's shown in "Article" content type. But it's not shown in the content type created by me. What could be the reason? Should I add it in the configuration? If this is the case, what form id I should add in configuration?

I having the same problem. I've added a content type of "guestbook".
I have checked the "Add CAPTCHA administration links to forms" box on the "admin/config/people/captcha" page. However, no new question is appearing on my "admin/structure/types/manage/guestbook" form page.
I tried entering the name of the form (guestbook, guestbook_form, and node_guestbook_form) in the box at the bottom of the "admin/config/people/captcha" page, but that did not work either.
Any suggestions?

I did notice that when I checked the "Add CAPTCHA administration links to forms" check box, I noticed that a captcha type box appear in my search block. It was hard to read as there are two layers of text on top of each other. The blue text read "Place a CAPTCHA her for untrusted user" The black text underneath it states "A: no challenge enabled".
There is no search block listed on the "admin/config/people/captcha" page.
When I uncheck the "Add CAPTHA administration to forms", the message in the search box disappears.

I was able to manually add the form name at the bottom of the Home » Administration » Configuration » People » Captcha form.
The format for the form's name is
_node_form. So in my case, the form name I entered "guestbook_node_form" in the text field at the bottom of the FORM_ID field group and the captcha challenged finally showed up on my add Create Guestbook content type form.

There you will see the CAPTCHA admin link.

However, I just tried it again and I notice that my guess book content type, while it appears in the Home » Administration » Structure » Content types list, it does not appear the Home » Add Content list.
So I'm going to have to figure out why my content type does not appear in the add content list / menu. Once I figure that out, I will respond back to this issue.
However, the manual method still works.
Also, when I turn on the Captcha adminstration Link feature, a message regarding captcha appears in my search box.

By default CAPTCHA's are not shown/showable on administration pages (page urls that start with admin/), unless you enable the option "Allow CAPTCHAs and CAPTCHA administration links on administrative pages".
But, this is generally not what you want: you want to put a CAPTCHA on the "node add" form (for general/anonymous users), not on the "node set up" forms (for site adminstrators).
This is by design: the CAPTCHA admin links are added to all (non admin) forms, such as the search box. Unfortunately the search box is so small that it looks very weird indeed. However, non-admin users will not see this.

I figured out my mistake and I have it working. Thanks for the help.
I did change the status back to active because I still have the Captcha button on the Search box when I log in as user 1. Is this normal or this is this a bug? I've attached a screen shot.

In case of such a small form/canvas as with the search form, things get wonky unfortunately.
I would not use this thread for this "bug". I created a new thread #1831652: CAPTCHA admin links look wonky on search form
Anyway, it is not recommended to leave the "CAPTCHA admin links" feature on all the time. I would only enable it when you're setting up CAPTCHA's
