I'm running into a weird situation where I have disabled the Image Gallery module, yet it still shows up in the /node/add/image form. Does anyone know how I could manually remove this reference from the database or if there is a way to theme the add and edit pages for an image node type?

Total noob moment...I had set a vocabulary for "Image Galleries" in the Category module. Removed and all is fixed.
