How to use survey.module - where to create actual survey form?

hello,
for the life of me, i can't figure out where i'm supposed to actually create the survey. i've uploaded both forms module and survey module (4.5). i'm using drupal 4.5.0. i'm not getting any obvious errors. i see the survey module and can create a survey (under create content), but it just looks like another way to submit a page, blog, or other kind of node, except that submission form asks for intro text and an url for the "thank you for participating in the survey" page - is there actually a place to create a form to collect data?? i'm sure i'm just not seeing something, so any help would be much appreciated,
