D6 module has error in if logic

Hi,

It looks like the D6 version (both stable and dev) doesn't work at all right now, because the logic for attaching the cookie handler only ever applies to the system_performance_settings form. The if statement for adding the settings fields needs to close before it starts adding the generic stuff that should be attached to all forms.

The attached patch fixes this up, or a new backport from the D7 release would also fix the problem.
