new question types - multiple "short answer" or "long answer"

There are cases where User needs to give many short (or even long) answers related to the same question e.g.

Name 4 uses of a xxx:
a.
b.
c.
d

In this example, it is better to have the fields grouped, as opposed to 4 separate questions.

Please has anyone done this in D6 or D7? What's the best way to approach it? Using an "Add more" button in configuration may be useful.
